  1. I-485 instructions say to make a copy of I-94 and include that in with the form. VJ says make a copy of the front and back of I-94 and submit with I-485. There is nothing on the back of my wife's I-94. Everything is on the front. Make a copy of front and back anyway? sorry for this type of question but we are pretty much ready to submit AOS and this kind of stuff...as you guys know...always seems to come up thanks all
  2. Hi all. My step son will turn 14 in about 4 months. Do we still submit everything based on him being 13 now? IE...no G-325a required for him...etc. Also, is USCIS Chicago Lockbox where we mail our AOS? thank you Don
  3. Might as well ask this (hopefully) last question while I am asking. Several of the forms have a blank for A#. I am assuming that is the alien registration number found on the I-94 and 797? Should we put that number on the form? I remember the K1 forms asking for that but we never put it in.
  4. Wow ok. I filled out one for my step son and printed it. Guess I will trash his G-325a. Thanks again Dan!
  5. Right....I thought about that too. Also, do we need to submit (2) G-325a forms per person like we did for K1?
  6. Thanks Dan. I don't think it will be a problem but you never know. We have filled out and signed every form with her New name except for I-693. but I-693 matches her passport so I'm hoping won't be an issue.
  7. Hi all. When we requested my wife's social security number, we had them add my last name as she wanted to take my name. When moved her maiden name to now be used as her middle name Should she be signing all the forms as her NEW name (which includes my last name) or the name from her passport? I ask this because she has signed all the forms with her new name except for I-693. The Dr had her sign I-693 with the name that matched her passport. will this cause us a problem? thanks all
  8. Thanks guys. I never thought this would be the form that confused me the most lol. Do I need to make a photo copy of the supporting docs for I-864 such as pay stubs and tax docs to attach to the copy of my wife's I-864? Also, on part 5 #21...do I count him as a dependant since he is being counted in #9? thanks again
  9. From what I've read, I dont list him on part 3 #9 but instead fill out a separate I-864 on him. My question is this. Do I list him as a dependent on #21 on my wife's I-864 and do I list my wife as a dependent on #21 on his I-864?
  10. Hi all. When filling out the I-864 for my wife, it asks in #9 if I am sponsoring any other family members. I assume I list my step son here. Question is...if I list him here do I also need to fill out an additional I-864 for him, including supporting tax and pay docs? thanks all
  11. Right. CCP processing took us 88 days. I emailed GUZ quite a few times and their response was rarely worded exactly the same. Hang in there
  12. Seems like it should be such a simple thing Larry but I could not convince my doctor that he could certify ONLY the vaccinations and that my wife did not need another physical. Thank God I found this other CS. The journey is never dull...is it? lol
  13. Thanks Dan. Great information! I just called another Dr. on the CS list and they were very accommodating and were very well versed that a second physical is not needed. They even said no appointment is needed and they are open on Saturdays.
  14. So are we better to go to get the vaccinations at the health dept prior to making an appointment with the civil surgeon?
  15. I emailed my Dr. and he said if she only needs vaccinations that it would be cheaper for me to take her to the health dept as their vaccinations are much cheaper than at my doctor's office. I still dont think he understands that he must sign it. I may need to call around to find someone a little more experienced in this.
  16. Thanks Dan. That's what we'll do. If not then I will find another Civil Surgeon.
  17. Thanks Dan. When I mentioned this to his office manager, she said if I bring in a copy of the medical exam she had the doctor might be able to waive it. This is such a pain
  18. Hi all. My general family doctor is also a US Civil Surgeon. I made an appointment with his office to take my wife there for her vaccinations. Since she already had her physical in March for the K1 requirement, I contacted the Dr. and his office manager and let them know that we wont need another physical. He said he is certain we do need another physical in addition to the vaccinations. Any one experience this before and if so, how did you handle it? We clearly don't need another physical but he seems to think we do. Any help is apprecitated thanks
  19. I agree Tsap. I contacted our senator and it took him nearly 2 weeks to find out what I was able to find out on my own in less than 24 hours.
  20. We had interview in March 2012 and our P4 was email only. No letter
  21. This is ALL great information guys. Thanks alot!
