According to letter_to_K1_or_K2_Applicant-May_09.pdf: Only these items are needed: Submit these items during your medical exam: ¡ñ Tuberculosis Classification Cover Sheet£»Form DS-2053 Medical Examination; Form DS-3024, Chest X-ray and Classification Worksheet; Form DS-3025, Vaccination Documentation Worksheet; and Form DS-3026, Medical History and Physical Examination Worksheet. ¡ñ Valid passport, four visa photos and interview appointment letter. Each applicant must have a passport that will be valid for at least 8 months beyond the visa issuance date. Every applicant must have a medical exam, regardless of age. These forms will be completed by the panel physician during the exam. However, reading through some messages here and messages from the 001 site, they said the Birth cert and such are needed as well? Have things changed recently? Are Birth, Policy, Unmarried Cert needed for the medical exam these days?

  7. Hum... I think Tetanus and diphtheria toxoids is the second one on the list, which is "Td". The Chinese is ³ÉÈË°×ÆÆ, which specifically says it's for adults. Is that right?

  14. We are having a problem getting a proper unmarried certificate. Our problem is similar to this http://candleforlove.com/forums/index.php?...c=20548&hl= The Start Date on the certificate is after my fiancee reached 20. The notary insisted that the certificate is good, but GUZ said otherwise. We are having a hard time right now...
  15. In P4, on the medical forms, the Chinese version asks for Case Number while the English version asks for Alien Number. Case Number sounds like GUZ case number, but it could be Alien Number as well. What is the correct number to use? The number starts with A (in NOA2)? or The number starts with GUZ? Thanks!
  16. P4's letter_to_K1_or_K2_Applicant-May_09.pdf says: Two copies of the DS-156 form in English and Chinese with original signatures(with Chinese name and telegraphic code) . I only see telegraphic code on the Chinese version. Is there anything special in terms of the signature? In the English form and Chinese form? Do we have to put in the telegraphic code anywhere in the English form? Thanks!
  17. I see. It looks like the single certificate differs from province to province. There is no place to sign for my fiancee. I am hoping we won't have any troubles. Thanks for the insight.
  18. The notary took that page right? Right.....she signed the paper at a local CCP office. Thanks for confirming!! I don't know why the other couple had the signature problem. It makes me a little paranoid. By the way, both my friend and my fiancee said the notary took the original and didn't give back. The kept it for record keeping. Is that what happened with you guys as well?
  19. I asked a friend. He came to the USA with K-1. He said, when he went to the notary, they gave him papers to sign. The papers are to state that everything is factual. But he doesn't remember if that signed page(s) are included in the white books. And he doesn't have the white books. A member here said that the white book was missing a signature so they got a blue slip. I am trying to prevent that from happening for us. My fiancee also said they asked her to sign for the factual statement. But the signed pages are not included in the white books. Can someone who just got the white books let me know what is correct? Does the white book include any pages that are signed by your fiance(e)? Thanks!
