Couple Questions on G-325A

The G-325A form I just printed out yesterday, on the right upper corner it says, "Exp. 05/31/09". Do any of you guys have a link to a G-325A form which is not expired? Or is it ok for me to use this one I have?

 

On this form, there's a "File Number" box, which should start with "A", Is it referring to alien registration number?

What about the "File Number" box on G-325A form? It starts with "A". I am not sure whether I should put my alien registration number. Because they asked for A number at the bottom of this form. Should I just leave it?

 

 

The name and A# are at the top, and in the box at the bottom. Don't leave anything blank

Standard answer for ALL government forms.

 

The date is NOT the form's expiration date, the OMB date is the date that USCIS has to complete a survey to determine the length of time it takes to fill the form out for the paper work reduction act, as mandated by the Office of Management and Budget.

When a USCIS form is due to expire, the form and any changes must be reviewed by the Office of Management and Budget (OMB). If a form expires while it is being reviewed at OMB, the form may still be used. Once a new edition is available, the USCIS website will be updated to ensure all useable editions are listed.

If it is posted to USCIS.GOV, or STATE.GOV then it is the most current form.
