The new one is (5) pages and requires some new information on it. The new form became effective 4/02/08, and the old form is no longer accepted!! Mark and family!! Link to comment

Great, now I'm thinking it'll be 4 overcomes the way luck is running I-693 (for a full medical exam, usually in Guangzhou) or the I-693A vaccination supplement are provided by the physician and submitted in a sealed envelope. If the Guangzhou medical exam I-693 is less than 1 year old, you do not need to submit a new one. Link to comment

If the Guangzhou medical exam I-693 is less than 1 year old, you do not need to submit a new one. This new form is a combination of both, the medical and vaccination form!! My civil surgeon told me that he was instructed by our Uncle this is the only form they would accept now!! George, you might be in under the wire?? Link to comment

What if I am a K nonimmigrant visa holder and already had a medical exam overseas? If you were admitted as a: A. K-1 fianc¨¦(e) or a K-2 child of a K-1 fianc¨¦(e), or as aB. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen, andC. You received a medical examination prior to admission, then-a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination. b. You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification. Be sure and take YOUR OWN copy of these instructions to the civil surge3on. Only need to have vaccination section of I-693 done. The full I-693 is for non-immigrants adjusting status in the USA from a NON-Immigrant visa that did not have immigrant intent like a visitor's visa. OR it is for a person adjusting status more than a year after the medical for their K-Visa, for example a K-3 adjusting status a year or so after getting their visa. Many Civil surgeons like to whack unsuspecting K-Visa holders with a full medical. Don't let them, if they won't do just the vaccination section, then look for a different doctor. This one will do the vaccination section by mail for $20 Dr. L. E.
