  1. Here’s what we’ve got. I do believe her vaccination record is incomplete, despite it saying that the “K Visa applicant voluntarily complete vaccination requirements”. She also was told to return for the second half of the MMR and varicella vaccines, but we had already left Guangzhou at that point and are now in the states. Thanks for the help as always everyone!
  2. Hey guys, I'm now in this section after a fairly enjoyable K1 process.... My questions is about this medical. I've done a fair bit of research but I can't quite answer my own question. My wife was pregnant during her K1 medical, so she did not get any vaccines. They did not give her a DS-3025 either. I see via the I-485 instructions... C. Fiance(e)s: If you are a K-1 fiance(e) or K-2 dependent who had a medical examination within the past year as required for the non-immigrant fiance(e) visa, you only need to submit a vaccination supplement, not the entire medical report... I don't understand if this means I need to fill out a DS-3025 or just the "Vaccination portion of the I-693". Which is the final page of the I-693. I definitely wouldn't want to mail in a I-693 that is totally blank except the vaccination page. Unless this is what is expected. (???) It also states that I do not need to include the "entire medical report". So it seems that all I need to send in with my I-485, as far as medicals go, is the vaccination proof. I don't even need to provide proof that a medical was done in China (I don't have anything to prove that anyway really) as the K1 visa wouldn't have been issued without it anyway. True? Thanks guys! I wouldn't have made it this far without this forum.
  3. Please post Name, Address and cost of Civil Surgeons you may have had a bad experience or good experience. For example a K-Visa holder adjusting within a year of medical for visa interview should only need vaccination portion of I-693 done NOT a FULL medical. Note CR-1 and IR-1 have this handled before Visa interview so wont deal with the Civil Surgeon in the USA. If adjusting from a visa type like a student visa then a FULL MED is required.   4. Hello All! My wife is here on K-1 visa, we got married one month after she entered the country. We are prepping for AOS and have a question about vaccination. She got her shots done in China and all they gave her was the yellow booklet, she did not receive DS-3025 form. In her situation, she needs to go to a Civil Surgeon (CS) to complete certain parts of form I-693 correct? Does she need to do a full medical? Thanks!
  5. Hi, When I received my K-1 visa I was living in the UK. The Varicella (chickenpox) was not available. I obtained all other shots and this was included with my medical. Those were all taken when I entered the country. Do I need to get the shot before I submit my I-485? Thanks Liz
  6. I was looking at a copy of ds-3025, and I was wondering which vaccines were actually required. It wasn't specific on whether it was documenting what has been done, or listing what was required. DPT / DT Diphtheria / Pertussis / Tetanus (Pertussis vaccine normally not given to adults) Td Tetanus / Diphtheria booster. Polio (OPV/IPV) Measles (MMR) Mumps (MMR) Rubella (MMR) Hib (influenza Hepatitis B } Varicella (Chicken Pox) Pneumococcal Influenza I know that Pneumococcus (Pneumonia) is only vaccinated for with people over 60 or so. Influenza is normally considered optional. I don't believe that Chicken Pox is a required childhood vaccination yet. Several of these vaccinations require a series of shots. I know that Hepatitis B requires 3 shots over a 6 months period. Polio also requires multiple shots. Should one check the vaccination status early and start updating them if necessary? ----- Clifford ------
  7. Hi all. My general family doctor is also a US Civil Surgeon. I made an appointment with his office to take my wife there for her vaccinations. Since she already had her physical in March for the K1 requirement, I contacted the Dr. and his office manager and let them know that we wont need another physical. He said he is certain we do need another physical in addition to the vaccinations. Any one experience this before and if so, how did you handle it? We clearly don't need another physical but he seems to think we do. Any help is apprecitated thanks
  8. Hi all. We have been back in the US and married for about 10 days now. As we are now looking to start working on the AOS, is there something I need to do prior to filing? One thing that comes to mind is do we need to start getting her name added to power bill accounts, phone bills, bank account...etc to show proof that she and I live together? Appreciate the help thanks Don
  9. Hi everyone: I appreciate your help. My wife completed her medical examination and all vaccinations in China (Guangdong Int'l Travel Healthcare Center) as part of her K-1 Visa. The hospital gave her a sealed envelope AND a bag full of other stuff; including Form DS3025 and a sealed CD which was separate from the sealed envelope. At the K-1 Document Intake, she gave them all things but they only took the sealed envelope. The person told her 'america might want to see the DS3025 and CD' when she arrive to USA. However, at POE, SFO immigration did not ask to see any medical so we still haver this sealed CD and DS3025. (she was expedited through SFO immigration in less than 2 minutes by the way. ....) Now, we are looking to complete the AOS and I am so confused on what to do. I understand that since we completed the vaccinations in China that we dont need to do it again here, but do we still need to see a civil surgeon to certify the DS3025 (and sealed CD?) and complete the I-693 just for vaccinations? Or do we just give the DS3025 (and CD?) with the AOS package with no I-693? This is so confusing because the i-693 instructions say we dont need to see a civil surgeon but it doesn't say what K-1 needs to give to the AOS. Also, I perused the pinned thread and the info there wasnt wasn't clear to me on what someone with in our situation (unsealed DS3025/sealed CD£©should do. Please advise. Also, assuming we need to see a civil surgeon, would her primary care physician from my health insurance = a Civil Surgeon, or do we need to see somebody special?
  10. I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.) The POE took her medical forms, so we don't have anything to submit with the AOS packet. Am I just to submit the vaccination I-693A supplement? Or do we need to get a new medical examination here to submit?
  11. We filed $1010usd worth of stuff (sorry) a few months ago. Then,we received a letter instructing to go get fingerprinted in Tampa. Then,we waited. Then,the EAD Working Card arrived in the mailbox and my wife started LEGALLY working a couple of months ago. Thats it. WHATS NEXT??? Have I forgotten anything? I assume the next thing to happen is a letter will come instructing to go make Medical Exam (in USA) should arrive? Theres nothing more I can do-right? Just wait for the Med-Exam instructions to come in the mail? My wife has started pestering me about this so I had better have an answer for her tonight! (I think we are supposed to just wait for that Med-Exam letter,but every time she gets around "new" Chinese friends,and they start giving her "advice" about her Green Card,she starts getting impatient and militant towards me.)   12. I have no idea what's going on with USCIS...... My husband entered into US thru K-1. As is informed, we just need to submit I-693A. However, after waiting for 4 months, we got a notice today that request him to submit the original DS-2053 (which was given to the custom when entering into the US) or HAVE THE MEDICAL EXAM AGAIN! Basically the reason for that is no medical exam could be located. We are living in NY. Anyone else has the same case? I guess we 99% will end up having another medical exam, which is really ridiculous!

Before filling out the AOS applications for my wife (K-1) and stepson (K-2) I had to pay over $300 at one of the USCIS civil surgeons in order for them to fill out the I693A supplements (the CS said each had to have some shots). They filled out the I693A and gave it to me (not sealed in an envelope). I sent in both AOS's and my wife has since been approved (will get green card soon). My stepson, however, has an RFE which states as follows: ================================================== Request for Additional Evidence (I-485): This office is unable to complete the processing of your Application to Register Permanent Residence or Adjust Status, (Form I-485) without additional information. Please submit the information requested. Medical Examination (Form I-693): Note: All medical examinations and vaccinations must be conducted and administered by a physician who is on the list of Civil Surgeons approved by the USCIS. All medical examinations MUST be submitted in an envelope that has been sealed by the Civil Surgeon. A review of the record indicates that you entered the United States pursuant to a valid K-2 nonimmigrant visa. All USCIS records relating to you have been reviewed. The U.S. Department of State medical forms DS-2053, DS-3024, DS-3025 and DS-3026 are not in any of those records. I-693 Medical Examination: Please submit a properly completed and signed Form I-693, Medical Examination of Aliens Seeking Adjustment of Status. The medical examination must be conducted by a physician who is on the list of Civil Surgeons approved by USCIS. Current regulations now require that the tuberculin skin test be completed by all applicants 2 years of age and older. A chest X-ray is required only when the reaction to the skin test is 5 millimeters or more. Immunization Form: Please submit a properly completed Supplemental Form to I-693, Adjustment of Status Applicant's Documentation of Immunization. The supplemental must be completed and signed by a designated Civil Surgeon. ================================================== 1) I thought that only the I693A (supplement) was needed, not the entire I-693. My stepson took the medical in Beijing China along with his mother before the interview. There was no I-693 given to either of them to bring to the USA. 2) Are they asking for all of these documents: DS-2053, DS-3024, DS-3025, DS-3026? They were given the DS-3025 which had their immunizations (which the civil surgeon transfered to the I693A). I don't know what the other forms are. 3) I'm sure he got tested for Tuberculosis during the exam in china. Not sure what to look for in the documentation he has for the medical. Does anyone know? In short its not clear to me what I need to send in (sounds like both the I-693 and I-693A sealed in an envelope). Not sure why or how I go about getting this done. Can I just go back to the civil surgeon and tell him he made a mistake and must give the I-693A in a sealed envelope? Are they asking that my stepson again get a complete physical even though he got one at the same time as his mother? Is it possible that they lost all his medical information from china and thus don't have those DS-xxxx forms so they now want a full I-693? If anyone has any insight please let me know. Thanks.
  14. I applied for fiance visa and arrived in USA in Jan. Before I came here, I had my medical exam that required by US consulate for visa interview. Now I am applying for the adjustment of status and Form I-693 needed to be filled by doctor. Does anyone here know whether I need another medical exam for adjustment? Thank a lot for kind help.
  15. My wife got here on a K-1 Visa, and had the required medical exam in China on September 24. We filed our AOS application on January 6. Now we got an RFE saying that she needs to get a medical exam and send in an I-693. I called the 800 number listed and they were not very helpful. They said we need to send a letter back with the RFE if we want to dispute the need for an I-693 - saying my wife is here on a K-1 Visa (shouldn't they know that?) and that she had a medical exam in September 2005 and we filed the AOS in January 2006 (again, you would think they would know all of this!). Now the way I read the rules in the I-693 FAQ - http://uscis.gov/graphics/i-693faq.htm - for K-1 Visa's if you had a medical exam within 1 year of filing AOS, you do not need to get a medical exam in the US - nor do you need to send in the I-693. We did get her vaccines here in the US and sent in the Vaccine Supplement form with the AOS. So I am wondering what to do. I don't want to hold up our AOS application, which within the past few weeks got transferred to the California office for "faster processing". Any suggestions would be appreciated. I guess we will try sending in a letter, but I hate to get yet another RFE a few weeks later saying we still need a medical exam and an I-693! Thanks, Fred

Hi all on 10-17-05 my so (K1) and her son (k2) recv notice from uscic (calif serv center) request for more evidence- pertaining to I485 application, my so's letter stated that they req, evidence for bonafide relationship, as well as I-693 medical. so's son's letter stated req. for evidence for marraige cert, and I-693 med. also. we stop by our local dhs office to clarify, and was told that being k1/k2 they dont need to repeat the medical, just need vacinations( I-693 suppplemental ),(which we did back in jun-05) ,because their medical was done in GUZ prior to their interview and placed in the sealed envelope ,which we gave to customs upon entry to usa. anyway,, now i dont know what to do or believe!! has anyone who is k1/k2 had to go thru this before? I am willing to get the medical's done again if it will satisfy things! but not needlessly! please some help!!!!!!!!!! anyone!!!!!!! feb-04 1st trip ,chengdu-engaged apr-04 2nd trip ,chengdu ,filed I-129f vcs may-14-04 1st noa,I-129f jul-04 p3 recv, sentback same week oct-04 3rd trip to chengdu, (to wait with so) dec-04 p4 recv, feb-2-05, interview,,passed!! feb-3-05, pick up visa (and get the heck outa GUZ!) mar-23-05, arrive home !! so and son apr-14-05, married apr-05 so gets SS card( in NJ its easy) apr-25-05 filed I-485,I-765, (for so and son) may-10-05 recpt notice of above may-23-05 recv. yellow paper req for additional evid(box checked submitt supplement to I-693 vacinations) box not checked for I-693 medical. jun-05 vacination complete sealed envelope sent to DHS lee's summit MO. jun-05 biometrics done for so and son jul-6-05 so and son recv work cards sep-22-05 recv notice from DHS (calf. center) satting that their I-485 applications have been sent back from DOS to serv center for further review ???? oct-17-05, so and son, recv blue paper from DHS(calf center) req for more evid as stated on the top of my post
  18. Ok I'm a little confused. I've completed all the forms for AOS except the I-693 which if I am reading it correctly means that Bing has to have another whole physical exam. If so what did I pay 100 bucks for in China? We have the Vacination records she got after visa approval, but don't they put all the medical records in the mysterious brown envelope? all we have is a little 3X5 booklet of her medical exam. Is this what we take to the civil surgeon along with X-rays?
