I found this had been a panic for me, I thought it was actual record check and a whole deal. I ended up just making a simple affidavit version, and took 2 routes: I had my statement and signature notarized (by my sister which was a nice visit home) then took it to the secretary of state office in Illinois to verify the notary signature, the sec of state process had nothing to do with me, it was verifying the notary! Then I took it to china consulate in Chicago paid the $20 and form, and submitted it at same time I applied for visa to go to China. I included an envelope to mail it to me, but they forgot so after emails and phone callls that went unanswered I did a very firm fax to the consulate and they called me first thing next morning and apologized and said they would mail it right out. I was worried so I also did the same process in Ohio and was ready to mail it to NY consulate if I needed. I never needed the document translated when we went to register for marriage, and the documents were fine. If you want my example I would be happy to share. poolman802@gmail.com
