Can this form be filled in Chinese? Does it have to be filled in English?

How is she going to fill in information about your family in chinese?

 

If she's going to do that part in english, should do the entire thing in english... All the forms should be in english is possible.

 

I pre-filled it out for my SO,she just signed it... which is another way...

 

the big problem, as lee has pointed out, is that the medical personnel who take all the forms will have their own interpretation sometimes of what is needed.. so you best to get it in english.

Question: The GIV-24 asks if spouse had more than one marriage or concubine please include. List all children from that relationship.

 

Do I write my ex-wife on this form and children from that marriage? If the children are over 18 do I still list them?

 

Do I list my parents on this form?

Yes you do list your children even if over 18, and from a previous marriage, they are still your children.

 

Yu filled this form out completely in English, and only Chinese Names were in Chinese characters.

Just a single question on the GIV-24.

 

Anyone know of a link to a "fillable" GIV-24 form?

 

If so, that would be great. I have yet to find one. My SO can add the written Chinese characters at a later time.

No fillable ones out there, the PDF you see on the web is just a scan of the paper form.
