  1. Check the expiration date on your K1 visa first. I am a K1 visa holder as well, and I just went to the California DMV to apply for a state ID. But they said after the expiration date on my K1 visa, they wouldn't be able to issue me a state ID, until I get my green card.
  2. SO the X ray was negative, no signs of any infection or disease. Now on to the I-485 Form, the directions ask for a bunch of initial evidence. They want Shuyans Birth certificate again? Police clearance? I understand the passport stamped w/ K1 Visa & the Marriage license? But we submitted all of this not long ago. They Kept a big sealed packet Shuyan brought when she arrived @SFO International in Feb 2009.
  3. So my civil surgeon emailed a CS coordinator from the department of homeland security. And the reply is, an overseas report is not acceptable to be submitted with a complete I-693 form. If the applicant had a positive TB skin test reaction of more than 5mm will also be required to undergo a chest x-ray here in U.S. And $40 I paid last time is what he charged me for complete the I-693 form. No more charges.
  4. We paid $40 for the administration fee today. Will go back with my HPV vaccine record next Monday and complete the I-693. No idea how much he would charge again tho. Will let you know by then.
  5. I want to thank SAMSONG, DNOBLETT, TOM&LING and RANDY. We couldn't do this without your help.
  6. We went to the Santa Clara Health Department this Tuesday and got vaccines of Tetanus, MMR, and also TB skin test. I had chicken pox when I was a kid, so I didn't get Varicella. We went to the civil surgeon office today with the I-693 instructions, and managed to skip another full physical examination. The civil surgeon wrote 'VH'(varicella history) on the form, he said a Titer test wouldn't be necessary. However, the civil surgeon still required us to get the HPV vaccinated. My TB skin test reading shows positive(55mm ), which means I would need a chest X-ray. I did the chest X-ray when I had the physical examination in China before the visa interview. I brought the X-ray tube with me today. And the civil surgeon thinks, since this chest X-ray I had is still within one year, it should be valid and we should give it a try. He copied my radiological report and enclosed it to the I-693 form. He said he would e-mail some connection in IRS and make sure. Do you guys have any idea whether this is acceptable?
  7. Shuyan and I have made an appointment w/ a US Civil Surgeon office they are saying we will need to get the HIV/AIDS test as well as Syphlis test, blood work and lab test's? ll Extra $$$ Are these tests required as well? We are going to go to the Santa Clara Dept. Health its a total of $330.00 for the 4 Vaccines and office visit. The US civil Surgeons office charges way more for these same shots, We will need the physical as well so the US civil surgeon will perform physical and complete the I-693 forms. Lastly We are a little late on completing all of this stuff is there a certain timeline we are married and SSN# was issued within the 90 days of Her entry to the US. Is there a deadline to have all these forms filed? Enjoying eachothers company so much we have put off a few things. Now its time to really get it all filed and sent in.
  8. I have called some US civil surgeons in my area. However, most of them didn't seem to have a clue about the K-1 vaccination required. And they all required the whole physical done. Any advice? Thanks!
  9. What kind of hospital/doctor should I go to get my vaccination record and how much would it cost? I don't have any insurance yet. Thanks for advice!
  10. Thanks a lot for your advice Sam.
  11. Hi, I am a K-1 Visa holder and I have got married 2 weeks ago. I am in the process of adjusting my status now. I saw a CFL member advised me to get Vaccination Record(Yellow Book) transfered to an I-693 by a US Civil Surgeon. I remember when I got the health check in Shanghai before the visa interview, the nurse there told me that K-1 visa holders should get the vaccination required in the United States. Can anyone tell me what is vaccination record and how exactly to get it done? Thanks a lot! Shuyan
  12. Hello, I am a K-1 visa holder, and I applied for a SSN after 3 weeks I arrived in the United States on 6th Feb., 2009. And I received my SSN 1 week after that. Then, I got married 2 weeks ago and changed my last name. I went to the SSA today and wanted to change my last name on my SSN. But the officer lady told me that my I-94 was going to be expired tomorrow. She wasn't able to issue me the SSN with my new last name. And she told me I could not change my last name on the SSN until I adjust my status(green card). I do remember some CFL members reminded me that SSN must be done within 76 days after my arrival in the United States. And I did so. I just didn't realize the last name on the SSN can not be changed after 76 days. Is there gonna be any problem for me using the SSN with my last name before marriage in the future? Can I get a California ID from the DMV with my new last name? Thanks! Shuyan
