AZwolfman
Note, I-693 is only needed if a K-visa holder did not do vaccinations overseas getting the DS-3025 Vaccination record at the visa medical sealed in a separate envelope, avoiding having to deal with a US Civil Surgeon all together. They pasted her visa into her passport and put the vaccination record in a sealed envelope which she brought unopened to America Edited May 14, 2013 by Randy W rw - fixed quote box (see edit history) Link to comment
NickF
Agreed. My own Dr. gave me a runaround and he is a civil surgeon. Avoid Civil Surgeon if you can. I did end up finding one that was very familiar with the process but took me a while and alot of back and forth with my doctor Interesting. When ChunMei came here, I called my primary care physician, learned that he's a civil surgeon. He saw her, did the required physical, and charged us for a normal office visit. Link to comment

You got lucky, unfortunately most get a big hassle by the Civil Surgeons, many refuse to do just the vaccination portion of the I-693, and attempt to change $300 or more for a full physical, which is NOT required for a K-1 to adjust status, K-1 got physical done in China for the visa, and only need vaccination record done on a DS-3025 or I-693, this is a big reason to get the DS-3025 done overseas at the same time as getting the required physical done, and have the DS-3025 sealed separately for later use when adjusting status, it avoids this possible hassle. I started a thread because of this: http://candleforlove.com/forums/topic/41344-us-civil-surgeons-and-the-i-693/ Link to comment
