I485 Form questions // AOS

Hello,

We have a couple quick questions regarding the I485 form. Lili came to the States on a KI Fiancee visa on Dec 22nd. We married on Dec 29th. Here is our questions:

Part 1.

Question - What is your current USCIS Status? What do we put here? K1?

Question - Expires on? What do we put here? The I94 expiration date?

Part 3.

Question - In what status did you last enter? What do we put here?

Question - Nonimmigrant visa number - Is this the control number on her visa?

Question - It says to list your present spouse and all of your children. Do we put my name here since we are married now?

Part 3. Processing Information

It says to list present and past membership with every organization. I assume they are asking for CCP information again, correct?

We then answer a bunch of yes or no questions. One question is about the communist party. We answered yes. Should we include our CCP letter of explanation again?

Lili's visa looks different but there is a red number that starts with a letter next to the petitioner (me). This is the correct number, right? It is not the control number at the top........I just want to be positive.

We have a little yellow booklet of shots, a white sealed envelope (I have no idea what is inside), and it appears to be some white receipts.......Any idea what I send and to who? With I485 info?

Did the med center in China do a DS-3025 Vaccination form and seal it in an envelope marked something like "For Immigrations"? If not, and you just have a small yellow book, you will need to visit a US Civil Surgeon and get this record transferred to an I-693, visit several to get best price, and avoid the ones wanting to rip you off trying to hit you for a full medical which is NOT needed. In September she went to Shanghai for her 2nd set of shots. For the second set of shots we have a receipt. We also have all vaccines recorded in the yellow book. Can I open this white sealed envelope?

I would highly suspect the GUZ med center did the DS-3025 and sealed it in the envelope.

 

It's your call,

 

You could open it to confirm that it is the DS-3025 vaccination record, and if it is seal it in another envelope, and write on the envelope vaccination record.

 

OR

 

You can assume that is what it is, and send it to USCIS with the I-485. If it is not, the worse case would be for USCIS to issue an RFE for a vaccination record I-693 done by a US Civil Surgeon.

 

Keep in mind the envelope is usually sealed to prevent tampering, if the envelope is ordinary, and has nothing to show being opened, then it's your gamble to open it and use a new envelope.

Correct the same A# that you noted on the I-94 card.

 

Should also be noted on the I-129F Approval notice which you will be copying and attaching to the I-485 Application.
