'Intent to Marry' form - P-4

Our interview (document hand-in) is scheduled for June 20. She received a form which states that she is free to marry and intends to marry _______________ .... It also has what looks like a place for a notary to fill in. Does anyone have any knowledge of this form and when it is supposed to be filled out? Does the consular officer sign it?

Well, I'll be flying to Gz on the 16th to support my SO and bring over a bunch of documents and supporting evidence. Everything has moved so quickly and painlessly so far , I'm hoping the interveiw will go the same way.

Fill out and turn in at doc turn in the day before interview, yes will be signed at the interview.

 

Not being notarized, the interviewing officer will sign it.

 

Note any form that has a notation to not write below a line like the GNI-2 or DS-156 form, you know the drill don't sign it until in front of the visa officer at the interview.
