ON the Guangzhou consulate site there is a link to the Ds157, on the form it specifically says expires 9/8/2008 can we still use it?

If the consulate has that form on their page you will be fine.

 

Note: The OMB expiration date is NOT the form's expiration date, it is the date by which DOS has to submit data to Office of Management and Budget to determine the length of time it takes to handle the form as required by the Paperwork Reduction act. The form posted was from the last survey, then current form is unchanged, and it's survey is due in 2011.
