i-693 for aos application

thanks randy, i learn something new every day. i wonder why uscis makes you do the 693 again? if i get an extra copy of the 693 from the exam in guz will the uscis accept it as part of my aos package i will turn in?

But your paperwork follows you around. The IO at the AOS interview will already have the I-693 from the GUZ medical exam. In fact, your SO carries all the paperwork from GUZ to the IO at the Point of Entry in another sealed envelope

5. Vaccinations (See Technical Instructions at http://www.cdc.gov/ncidod/dq/civil.htm for list of required vaccines.)

is for the vaccinations. This part only (on a new I-693) will be filled in by the US Civil Surgeon. He will give you a sealed envelope with the new I-693 in it.

 

Your AOS file will contain 2 I-693's = one from the medical exam in China, and one from the US Civil Surgeon for only the vaccinations.

dale7570, Didn't you have to get the 693 done for the interview in GUZ?

I-693 is not done in foreign country for visa medical it is only used when a NON-immigrant adjusts status in the USA.

 

For visa medical the forms used are DS-2053, DS-3025, DS-3026, and DS-3024, all Department of State forms.

dnoblett, Now I think I'm more confused than OP. :blink: I thought he was filing K-1 and was trying to get ducks in a row for later AOS filing. Ergo my question. Now, did I read correct and the suppliment vaccination form is included with medical in GUZ? If so, you will still have to see a U.S. Civil Surgeon IF you need additional shots to finish off treatment.

For K-Visas Vaccinations are NOT required, this is done later when adjusting status.

 

My wife waited until AFTER being approved for visa, and then visited the madical center and got the required vaccinations, the medical center gave her an international vaccinations book (A passport sized Yellow Book", after we married in the USA, we visited a US Civil Surgeon, and had the record "Yellow Book" transfered to an I-693 vaccinations record in order to adjust status.

 

Again, for K-Visas the vaccinations are NOT included with the medical in GUZ.
