It indicats as follows:" this is to certify....has committed no crime against the law till dd/mm/2003. The notarary office told me it is a general way to write like this. Do i also need to have a cerfiticate that says i did not violate the law from may 2003 to aug 2003? I guess not..but not sure. Does anyone know that? Thank you.And also on the medical check up paper, there are information such as case number, name and etc. Do i need to fill them out myself or should i leave those blanks for the doctors to fill out?
