Question about form 169

For the form 169 that my husband is filling out, does he need to fill out both the English and the Chinese. He's a foreigner and neither one of us know Chinese so I was just wondering if he reallly needs to fill out the Chinese part or can he just leave it blank, since he's filled out the English. Sorry about all the questions. We'd rather be safe then sorry. We want to avoid mistakes if possible so we don't get it sent back to us to correct or so the process is not prolonged. That's all.
