joining the P4

I am traveling with my soon to be laopo and a few of my students from the states. a text-message came in yesterday--her family has the P4 at the apartment. yeah! just saw the scan of the top letter. August 1st.

 

We are going to end up in Beijing in a week and it seems the best place to take the physical given our plans. her mom is going to bring up the entire P4 packet to beijing when she joins us, but my question is:

 

are there any forms or papers that we need to bring to the physical that are not in the P4 packet.

- Passport & ID

- Four passport photos

- Money for exam (you can do pickup SAME DAY if expediting for 100 yuan and your there first thing in the morning; call ahead to confirm, if you want this..)

- Money to pay visa fee (you can do this after the exam and prior to pickup.. You can try to ask the hospital for closest location)

- Make copies of the blank forms prior to filling out.. if you have a mistake, the hospitals are not required to have blanks on hand...

- Consider getting vaccinations. If so, bring any prior proof of shots.. although it's cheap enough if you don't have any. Make sure to get the Yellow International book, it should show english (even if abbreviation) for the shot.

Concerning the photos, the acutal number needed is 6.

 

1 - medical Exam paperwork

4 - DS-156 & DS-156chn

1 - Vaccination paperwork
