My SO arrived at BJ west station, took 1.5 hours by taxi to hotel near hospital. Next morning, went early to medical exam.

 

HOSPITAL:

First floor, two rooms. Exclusively for physicals for visas.

 

Must arrive before 9:00am¡­ for same day service (no additional fee); 4:00pm pickup

 

NOTES FOR THAT DAY:

8 people present for physicals (at least 1 other fiancee from 001) ;

 

- Check-in

- Pay medical fee

- Physical (eyes, weight, height, draw blood, xray, check stomach, etc)

 

Time spent at medical facility: 2 hours

 

 

NOTES ON PAPERWORK:

- Hospital made copy of beneficiary's passport; put in envelope

 

- 6 passport pictures were requested; Hospital = 2 , 2 for DS-156 english, 2 for DS-156 chinese

 

- Would not accept computer print of DS-156, DS-156K, DS-157; made write in handwriting

 

- GNI-2, GIV-24 accepted as computer printed... My SO explained that GUZ never sent them to her.. only I did via computer (already filled in), she had no blanks on hand. ( I told her she should of said that for the other papers... but she had followed my instructions to bring blanks)

 

- Medical Staff only reviews forms are correct ones, not necessary that the information input is correct.

 

 

BANK:

 

Afternoon, she went to Bank, about two traffic lights, and paid 830 yuan

On your computer print DS-156, did you puy your name and signature on line 40 "Application prepared by:" ?

I did... but when they made my SO do it with her handwriting, I have no idea if she put it in.

 

Originally, for the signature, I wrote my name (in print), and in parens stated, "(sent in email)"
