No certification of legal capacity and ... form?

Hi all,

Certification of legal capacity and intent to marry isn't in my P4.

Where can this form be dowloaded?

You can get this at Certificate of Intent to Marry.

 

The only thing you can do with this form is write in your SO's name, you will sign this at the consulate.

This is a bad link, is there another one to print out and have in the kitchen sink or is this something they will give to her and she signs there?
