Docs required for children of divorced Perents

What documents are required to obtain a K-4 or CR-2 visa for children with previously divorced parents. My SO currently has her divorce record and a settlement document from the chinese courts. The settlement document assigns my SO full custody of the Son, the document also addresses that she (my SO) and son may move to another city or country.

Is a special permission document required from my SO's former spouse to give permission to move the son to the USA.

 

If So, Can the Visa unit give guidance as to the content of the document to meet this requirement.
