more I-485 questions

I'm looking over the I-485 instructions and have a few questions:

 

It says for K-1 that we need only submit a vaccination supplement, but does not define what this is. Where do I find out about this?

 

How does one answer these questions:

"current USCIS status?"

"in what status did you last enter?"

 

She just arrived on her K-1.

The civil surgeon should have the vaccination supplement, but you can also take a look at http://uscis.gov/graphics/generalvac.pdf to get some additional information.

 

I don't recall specifically what we put for Jingwen's current and entry status, but I'm going to guess I put K-1 for both since that was the status listed on the I-94

The I-693 (I think that's the right number) vac supplement is something you get from the civil surgeon. You don't need this for the I-485 but you must bring it to the AOS interview.
