Missing Supplemental Form

I just received the DS-230 from NVC, for the CR-1. My wife is in China. However, I was expecting to see the Chinese Supplemental Form, but it was not included, nor was it on the checklist.

 

Did NVC stop sending this, or do you think they made a mistake? I could easily send one, if you think that it is necessary.

For my fiancees K1, DS-230 Part I was included. But for the P4 package, there was no DS-230 Part II. Perhaps this have decided the form is of little value.

I think you need the supplemental form. It provides the info to send the p4 to your wife in China. Use the one that is returned with the 230 to the NVC. I think you can get it from links and resources here. I would also call the NVC and talk with someone there.
