I-693 suppliment

My wife is a recent K-1. We are in the Dallas TX area and the civil surgeons I have called (both of them) all seem to want to do a complete new I-693. They said the cost would be $50-75 plus vaccinations. They don't seem to understand she already had this done in China for the K-1. I guess she still needs followup to one or two vaccinations she had in China.

 

What is the suppliment I-693 ? Is it a different form?

How do I explain this to the civil surgeon?

Any specific recommendations in the greater Dallas area?

If your wife had her immunizations done in China she should have a small yellow booklet with her immunizations recorded in it. I took this booklet to our local civil surgeon and the office nurse filled out the suppliment to I-693 and sealed it for us to return with AOS paperwork. All was done free of charge - maybe I'm just lucky!
