dnoblett Posted April 17, 2012 Report Share Posted April 17, 2012 Since you live in Phoenix, I would suspect the Passport Agency located in L.A. handled the issuance of it. I don't think it much matters what you put down, so I would simply indicate the nearest agency. I GUESS WE COULD PUT THE HOUSE IN NANCHANG.I would list the first place you do plan on staying, on each of my trips, I did have at least the first hotel I was going to stay at booked, so indicated that. Note: that's what under "Authority" in your passport. If your passport was issued recently in the US, you may simply put down: US Department of State; If your passport was issued overseas, you need to indicate the country where it was issued.
