Blue slip

I got a blue slip too, however 2 items were checked. I am about to submit the required statements with supporting evidence. The blue slip requested a return address. Does anyone know if the address should be in Chinese, English or both? Do I need to include a self stamped return envelope for the reply?

 

Any input would be much appreciated.

Chinese characters, and Pinyin.

 

When turning in the requested stuff at the consulate, there should be China-Post stand in the hallway, postage fee can be paid there, and this will cover the packaging, this was how it was for our visa interview.
