Medical info for interview

Hi Guys

We have our interview date, Aug 26th. My question is will GUZ send us a Packet, #4 I think, with the required medical forms for my wife and daughter? If not, what forms do we need to download and print OR does the hospital have the medical forms we will need?

The consulate will mail out the packet 4, it will contain the required forms, and invitation letter which is needed to enter the consulate on the appointment dats.
