In the 3rd part, line 1, "date of intended departure", can I just leave this part blank? If I do need to put a date, what is the time frame? 6 months, a year?

Nope, you can leave that blank, K-1 waiting for AOS can get an AP doc valid for up to a year, with travel anytime during that year.

 

AP is an interim benefit that you are paying for when you adjust status.
