There isn't really much to fill out, which may be why it's not provided as a "fillable" form. OF-169 consists of confirming which supporting documents you are going to provide when you interview as well as your name, address, and case number. That's all there is to it. All of the examples I've seen have been completed by hand. Yep just a bunch of square boxes to check off, and then sign the form. Note: There is a box for "Military Record", if served in the PLA, don't bother checking this box, you cannot get the military record anyway.
