P4 Medical Forms, Do we need to fill out anything?

For the P4 medical forms, do we need to fill out my fiancee's name and case number and so forth, or will the hospital take care of the whole thing?

If I remember correctly, My Yu filled in her name and case number on the forms, the medical center handles the rest.
