dnoblett Posted July 20, 2009 Report Share Posted July 20, 2009 We applied using married name because that is what she wanted on her green card. There is a line on the form that asks for name on I-94, this is where we put maiden name. G-325A has a line for "Other names used" this is for maiden name. If you file using maiden name and then decide to later change it to married name after getting the card, you will file I-90 and pay a fee $370 Link to comment
