clayton2103 Posted January 21, 2008 Report Share Posted January 21, 2008 Late last week, after completion of the immunizations for my wife by the local civil surgeon, we arrived to receive the results.We were given a copy stamped "copy" of the I-693 supplement with his signature and also a standard white envelope with the name of the medical practice and doctors name as the return address, wife's name hand written on the front and an "Original" stamped upon it. On the back of the envelope it is stamped "Confidential" several times, his signed signature and then sealed over the top of all with tape. Is this standard procedure? If so, how do I include this with my I-485 AOS paperwork that I intend to send out this week? Thanks.Clayton Link to comment

david_dawei Posted January 21, 2008 Report Share Posted January 21, 2008 I didn't attach to paper... instead, I bought one of those clear 8.5x11 holders (already three hole punched) which paper can be slipped down inside... and put the envelope in this.... although you might clip or staple the top opening so nothing slides out if upside down... I will be a few days behind you, I plan to mail out ours on this coming Friday, January 25, 2008, one year exactly from the date of sending out our I-129F.I will be following your timeline closely so I can see how and when we should expect what's next.
