totup Posted January 31, 2007 Report Share Posted January 31, 2007 I am getting all the paperwork together to send in for my wife's AOS.She came here from Beijing china on a K1 Visa. From what I've read itseems that I don't need a full i693 since she had the medical in chinabefore her interview. I only need the i693a supplement which is forvaccination information. Is this a form that I need to find and fill outmyself? I'm looking through all my wife's papers that she brought withher to the United States and I don't see anything called a i693a. Whatshe does have that relate to her medical exam are the following: 1) a chest x-ray2) A small brown book (similar in size to a passport) labeled: "Certificate of Health Examination for International Traveller". It has general health evaluation (like Heart: Normal) with stamps, signature, and date from her medical.3) Another small yellow book (similar in size to a passport) labeled: "International Certificate of Vaccination". It has stamps, signatures, and dates for the vaccinations she took during the medical.4) Document DS-3025 "Vaccination Documentation Worksheet" which was signed during her medical. It seems to be a summary of the yellow book from (3) above. Link to comment
Randy W Posted January 31, 2007 Report Share Posted January 31, 2007 Take the yellow book with you to a civil surgeon - he will fill out the form I-693a and put it in a sealed envelope for you to turn in with your I-485. The I-693a vaccination supplement is also available on the USCIS website (try here) - you can fill out your information ahead of time. A list of civil surgeons may be found there as well.
