hey guys, sorry to bother but i do have a question here, anyone of you remember the original K1 apply forms you filled out before the P4 asked your fiancee to provide all the information about over 6 months resident since the 16th birthday? or just last five years residency info? i am filling out the P4 forms and i can't find my damn lawyer!!(oops, sorry for the d word:))

I think you want the OF-169.

not really p3 forms, i am filling my p4 forms currently, just wanna make sure the very begining forms didn't including my residency information since my 16th birthday but just my last five years residency info:)
