Thanks for all your helpJay The notarial booklet is sufficient. The separate affidavit would be appropriate if one used another method of notarization than that available in China. For instance, a petition might be filed for somebody who is already in the US and has possession of the original Chinese documents. They might then take them to a certified translator and get the translation notarized. IN CHINA you get the Notarial booklet. Link to comment

We usually ask for a notarial divorce decree. It will be in the standard white notarial booklet, have an official red seal, an English translation, and an attestation to the true translation.
