SS Card and Medical/Vaccinations

The necessary shots:

1) tetanus

2) MMR

3) chicken pox (Varicella)

 

You'll need to find a civil surgeon which will fill out and sign off on the vaccination supplement (I-693)... but this is where the three ring circus starts... and you'll probably call 10 of them saying they want a physical or paper proof of chicken pox (some will look at her arm and some will not) or a second booster for the MMR (and some don't mention it).

 

Spend the extra time to track down one who will do the least amount of work for the least amount of money !!
