(All forms cited below are available on line at<http://travel.state.gov> or at the Consulate) 1. Provide two (2) recent passportphotographs (2" x 2" = 5 x 5 cm). They must be with a full frontal view(without head covering), and light background. 2. Completed passport application form(DS-82 for adults and DS-11 for children under 16 years of age). 3. Previous passport will be canceledand returned when the applicant picks up his/her new passport. 4. Original or certified copy of BirthCertificate for minors under 14. 5. Pictures showing child's facialdevelopment from the year when the expiring passportwas issued to the year of application for minors under 14. 6. Required fees total US$67.00 foradults, and US$82.00 for minors under 16. Please note that Effective April 8, 2002, all passport applications must be forwarded to a domestic U.S. passport facility for issuance. Currently it takes about two weeks to issue a passport. Please also note that Effective July 2, 2001, the law requires that both parents must appear and sign the passport application of children under age 14. If only one parent executes the application, that parent must submit evidence of sole authority to apply, otherwise submit the other parent's notarized Form DS-3053 or notarized written statement of consent (including the child's name and date of birth, as well as parent's identification information or a copy of his/her ID) authorizing passport issuance for the child. A third-party in loco parentis applying on behalf of a minor under the age of 14 must submit a notarized written statement or affidavit from both parents or the parent having sole custody of the child authorizing passport issuance. ** For additional pages to you current U.S. passport.Form DS-19 must be used to request additional visa pages. U.S. passport holders over age 14 must personally sign the form but do not have to come in person to request this service. Instead, they may send someone else with the application form and passport. An authorization letter is required. For minor children under 14, both parents must appear and sign the passport application. Child's original of certified copy of birthcertificate must be presented.
