P4 forms

Can i go ahead and use the cfl forms on the cfl site instead of waiting for the forms from the actual P4? Do any of the medical forms need to be filled out before hand such as name, etc.

 

I havent really looked at them very thoroughly yet, but sure would like to get ahead of them.

You are best to either order a set of forms yourself or download them from the official DOS or GUZ web site.
