We used a single I-751 form for both my K1 wife and K2 daughter. However, you must include a separate biometrics fee for each person. That fee I think is currently $80.00, but of course it's important to check the form as the prices change from time to time. You must file the form within 90-days of the expiration date that appears on the green card.
