During our process, we might have done something wrong and lost the birth certificate for my wife. As she was born in the 70's in Guangzhou, where can a new birth certificate be issued?

At any PSB office, as long as you have the hukou book. It's best to bring her parents' birth certificates in case they are asked for.

 

Where did you get the first one? It's best to go to the one for her hukou.

 

Actually, Jiaying only ever got one birth certificate also and never needed a second, even in the U.S. But, if you can reasonably get an extra, it's a good idea to do so.
