I129F and G325a - Other names

Hi, need some quick help.....

 

When my fiancee was 7 years old (20 years ago?), her parents divorced. Because of the divorce, her mom changed her name. Do she need to put her formal old name in "Other Names Used"? If i do need to write the former old name, do I need to to mail any notarize documents/statements relating to this "old name"?

 

Her passport, hukou book, notarized birth certificate, and etc. all uses her currect name.

Other names used is for prior names as in prior married names or maiden name. You can provide the prior name, and will not be required to provide documentation of that prior name, unless it was a prior married name in which case marriage/divorce cert shows this.

 

I would note the prior name and leave it at that.
