G-325A Help!! AOS

Is only one G-325A original copy needed when filing I-485, I-693, I-765? Is it only needed for my SO?

 

In the field "This form is submitted in connection with an application for:" do i check off "Status as Permanent Resident" or "Other (Specify):?

Adjustment of status is an application. There is no petitioner / beneficiary. So only one G-325A from the applicant.

 

Status as a permanent resident. This is what AOS is, adjusting status to a permanent resident.
