GIV-24 question 8

I was wondering about the GIV-24 question 8. I have an estranged brother who has legally changed his name. I was wondering for the question 8, do I put his new legal name, or his old name (so that we have the same last name). I also am not completely confident where he lives now. I have a PO Box address for him though. Should I put that done for his location? Or should I just write down the province/country he lives in?
