Husband got sick to his stomach because we still need that damn Japanese police report. AHHHHH! I thought I was freaked out before but NOW I'm freaked out!! Oh yeah, my DS-230 only had 1 page, back and front...even though it says that the form is 4 pages. Is this a mistake? Can I print it out from the internet? What needs to be sent back to the Consulate? The DS-230, the checklist? Does the Affidavit of support also have to be sent in already? OMG my tax transcripts! OMG! I'm only good with knowing K-1 stuff so I'm not sure if you need to fill out all of the Ds230 BUT IF you do here's the link:http://www.state.gov/documents/organization/81807.pdfYes, just the DS 230 and checklist go back. The I134 you bring with to the interview. Tax transcripts don't take long to get...I got mine in less than 10 days from when I faxed the request to the IRS. Link to comment

You can download all the forms, plus return them when your ready return what it tells you on the instructions to return, different for different visas I believe and I'm no expert on DCF. I'm devastated because if we could send that damn DS-230 in now, our chances of making it back to Hawaii by the 4th of July are pretty good...we are having a HUGE family reunion at that time and planned to host a dinner/wedding celebration if we made it in time. On the DS-230, there are some things that don't apply to us, such as court and prison records, military records, custody documents, resumes, and visa extension notice. My question is, when we send this off, do we check those boxes even if they don't pertain to us? Or do we leave them unchecked? I don't want to check them and give GUZ the impression that we need and have them, but I don't want to not check it and give GUZ the idea that we don't have them and need them!??! Link to comment
