A friend of mine emailed me wanting to know where to get this form to file with the I-751. I went to the USCIS website and read the instructions for the form and it says you need to include it with your petition. My wife and didn't need to do this when we filed a 14 months ago.

Yep, only needed if filing overseas since there is no USCIS field offices overseas able to do biometrics.
