The first box has "In re:", "Date:" and "File No." How should I fill them out? Or it is for official use only. Thank.

Can I ask are you getting an attorney ? why ? if so they can fill it in, you can just sign it.

While these were in regards to AOS they may help, but it appears that you would like to add the G-28 to the I-129F Petition paperwork. I'm not sure if that is the case.

I'm not getting an attorney. I just want to give a trusted friend the power of attorney so that she could respond to an RFE, if any, while I'm in China.
