Family composite Sheet in P-4

My MM got her P-4 (for her k-1 application). She told me that there was a "Family Composite Sheet", which needs my family's info and my signature. I want to know:

Is my signature really needed? I remember that when I turned in my application for her k-1, there was a sheet required her signature, but I was told that it was not necessary after I called INS.

 

If you had similar experience, please let me know.

No. your signiture is not required. But your family information is needed. In fact, if she read the form closely, she will find out this. The information about you is stated as MY SPOUSE or Fiance ... Your MM needs to sign this part.
