Civil Surgeon - I-693 Supplemental Form

I just discovered that our county health department has made arrangements for the I-693 Supplemental Form. You go in and a nurse reviews your documentation, makes a photocopy of the passport and completes the form. Then every Wednesday afternoon a Civil Surgeon comes in and reviews the information collected and signs the paperwork.

 

All of this for $10.

 

I'd suggest you check with your local health department and see if they have something similar.

Such a bargain! Don't tell my wife. She still remarks how I paid less than any of her friends.

Yeah, I told my wife we would do this in July when our son arrives and she remembered what we paid for hers and I got The LOOK. :greenblob:

It was safer to tell her now than to wait and let her discover I kept it a secret, I just don't think I could survive that look. :huh:

Too bad the arrangement's not universal, but Lee, I would have thought you've mastered to fine art of shading by now.

 

"Lao po, I called our civil surgeon today and yelled at him for charging us so much for your vaccinations. He said he'll come to county health and do our son's for only $10." ;)

Lee, wasnt the I-693 Vacc supp supposed to be sent in with the initial AOS application? I was just reading the FAQ's where she needs to go to a civil surgeon to have it completed and then we send it in with the AOS app.

 

I am a bit confused........BTW, we are K1 if that makes a diff.......Ken

You are correct, it is a good idea to send it with the AOS paperwork, too many people started getting RFE's for it this past year.

 

We did the wife's at a civil surgens office for $50, but come to find out the county has an easy way to handle this. Seems like most of the immigrant community is using the County health department for their needs in our area.

 

So when our son arrives in July we will choose the $10 option. :huh:

Thanks, very helpful. I in fact just put together a list of C.S.'s in our area and was just getting ready to call today when I saw your post. I think I will call the county now and see what they say !

Ok, for anyone in Denver, here is some info regarding the I-693:

 

I called several places on the USCIS list for civil surgeons. Most are regular doctor offices and when you first tell them what you need, several were 'huh'? Most never even heard of a K1 visa and were giving me outrageous prices for the complete physical and I went crazy trying to explain she didnt need the physical.

 

Anyhow, one finally referred me to Denver Occupational and Aviation Specialists and I just learned they can complete the Vacc Supp for the I-693 for only $20 (needed shots are extra).

 

The # is 303-373-4456. They are located in B;dg 200 at the Crossroads Distribution Center on Havana St, 2 bks south of I-70. They take walkins M-F 8a to 5p.
