DS 157 in P4 Interview Packet ...Questions

SO in China received her P4 packet which included 2 differently titled DS 157....One is titled as: "FOR APPLICANTS 16 YEARS AND OLDER" The other is the one I am familar with and can be downloaded on the website "U.S. DEPARTMENT OF STATE SUPPLEMENTAL NONIMMIGRANT VISA PPLICATION"

 

My question is who fills the first one I mention above? Her son who is only 14 or my SO . Why GZ send it ? Never seen this form before has any one here ???
