Alien (case ) number?????

Just a quick question, on the medical forms it asks for the alien (case) number, do i put the GUZ case number or the alien number assigned from the 1130 NOA2?? Thanks we leave in the morning for Guangzhou, all night on the train...uhgggggggggggg

We left all the medical forms blank until arriving at hospital. The hospital provided direction on what was to be filled in prior to the exam

Use the GUZ number, i.e., case number.
