"A" number?" I think she should write, "US Citizen, born xxxxxx" and maybe include my passport number. What do you think? Link to comment

Forget about the passport number, if they needed it they would ask. I haven't quite figured out the value of this form unless this is a list used to deny temporary visas... This is just another case of one form being all things to all people. Would you please pass me the round peg, I found a square hole it might fit. Link to comment
