I went through the same confusion. So I can tell you what a lawyer told me:

 

Yes, you need a separate G-325A for the under 18 yo stepchild. This is for both I-130 and I-129F. The instructions for the I-130 do not say it is required, but it is. Also, if you ask a human being at INS, they will tell you it is not needed. WRONG!

 

Yes, you need to send an additional check for the filing fee along with the G-325A.

 

No, you don't need biographic data for the child.

I didn't file the G-325A with my 13 y.o. stepdaughter on the I-130 and it was accepted and later approved.

I did file the G-325A when I mailed in her I-485. It was accepted and now waiting for an appointment date sometime in the very distant future, in a galaxy, far, far away!...............:lol:

 

There is no filing fee involved with the G-325A.
